Program covers up to two cleanings, exams and x-rays per year; up to two fluoride treatments per year; one sealant per tooth every five years; and dentures, partials, crowns or bridgework once every five years. Children up to 18 years of age (or full-time students up to 24 years of age) are eligible under the family plan.
